This playtest feedback form is for game designers, testers, and teachers running test sessions for a board game, card game, or digital game. It includes fields for build or version, date, tester, and observer. The main section is an observation log with columns for time, what the player did, what they said or felt, and whether there was an issue. It also includes a question section with space for answers to prompts like what the goal of the game was, what was most fun, and where the player got stuck or confused.
